OSP is actively working with BCBSM and other national agencies to prepare our physicians for designation as Patient Centered Medical Homes (PCMH).  In support of these efforts, OSP has developed a comprehensive PCMH Toolkit.  This path is challenging, but together we can achieve PCMH designation.
